Care management and transportation
services are provided by a partnership between the Fairport/Perinton Senior Living
Council (SLC) and the Fairport Baptist Homes Caring Ministries
(FBH). Sunday Dinners are provided by the local churches.
Care Management (Senior Options for Independence,
or SOFI)
Experienced Care Coordinators offer assessment, information and
options that enable older persons to continue living safely and independently at home.
Includes home visits and assessments, designing a care plan, assisting with
community referrals and arranging services and follow-up to determine if needs are being
met. There is no charge for SOFI services. Donations are appreciated, but not

Van service is available to Seniors Mondays
9 AM to 1 PM, T-W-Th from 8:30 AM to 3:30
PM, providing transportation for doctor appointments, grocery shopping, hair
appointments, banking and other errands. There is no formal
fare, but a suggested donation is $2 each way, or $4 for a round trip.

An individual driver service is available for non-emergency medical transportation
outside of Fairport/Perinton and/or outside of Elderbus hours of operation.
Service is provided at varying times based on availability of
volunteers to drive. We need a minimum of 48 hours
notice for all requests for a volunteer driver. The service is
free, but donations are accepted for transportation
programs. Suggested donation: $5.00 each way.

Mall
Shopping
Transportation service to Eastview Mall is provided the first
and third Fridays
of each month from 10 AM to 2
PM There is no formal fare, but a suggested donation is
$2 each way, or $4 for a round trip. The shopping bus is provided by the Town of
Perinton.

They offer a free and nutritious meal and fellowship. They are offered at
12:30 PM every Sunday. The sites are handicap-friendly. Deliveries are
available for those who are home-bound by calling Mark Renfro at
223-0963.
